Inclusion Criteria

1. patients has more than 60 years old
2. Patients who received two or more medications
3. Patients taking continuous medication at Diabetes clinic, Hypertension Clinic and Cardiovascular disease clinic

Exclusion Criteria

1. Patients with cancer disease or terminal stage.
2. Patients with dementia
3. Schizophrenic patients
