The feasibility of increasing inpatient rehabilitation therapy duration by implementing an additional self-directed independent exercise program: My Therapy.

Increasing the dosage of occupational therapy and physiotherapy during adult inpatient rehabilitation improves patient and health service outcomes. This study aimed to determine feasibility of increasing this dosage with independent tasks and exercises outside of supervised sessions, called the 'My Therapy' programme. We compared two groups of patients who received usual care inpatient rehabilitation. The intervention group also received My Therapy. The patients were hospitalised adults with frailty or musculoskeletal conditions. My Therapy was prescribed by the occupational therapist and physiotherapist. We wanted to understand the percentage of patients that would participate in My Therapy, how much patients would participate, if there were and adverse events related to My Therapy, as well as additional patient and health service outcomes. My Therapy was offered to 116 patients. Participation in My Therapy was achieved by 72% of the My Therapy group, who averaged 14 minutes of daily practice. A minimal clinically important difference in function was achieved for a significantly higher portion of the My Therapy group (22%, 26 of the 116 My Therapy patients) compared to usual care (10%, 9 of the 89 usual care patients). There were no adverse events, safety concerns or group differences for other secondary outcomes. My Therapy was a feasible and safe way to increase the dosage of inpatient occupational therapy and physiotherapy rehabilitation via independent practice.

Patients admitted to Cabrini Rehabilitation (either Hopetoun Street or Glenhuntly Road) sites under the Orthopedic Stream or Reconditioning Stream.
Patients identified as having a cognitive impairement: Past medical history of dementia or current history of delirium.
Patients who do not speak English
