- •Diaphragmatic dysfunction, history of neuromuscular disease, continuous infusion of neuromuscular blocker, pneumothorax/pneumomediastinum, esophageal varix, hystory of gastric or esophageal surgery, pregnancy, ready for extubation within 48 hours, disagree with th e study
