Inclusion Criteria

1 Patients with diagnosis of Schizophrenia as per ICD 11 and presence of Obsessive Compulsive Symptoms as per Yale Brown Obsessive Compulsive Symptom Checklist

2 Patients having capacity as per Section 4 of Mental Health Care Act 2017 to give written consent or Nominated Representative agreeing to give written consent when the patient does not have adequate capacity

3 Patients on stable dose of antipsychotic medications for the past 3 months (no more than 50% reduction or increase in antipsychotic medications)

4 Age 18 to 65 years

5 Any gender

Exclusion Criteria

1 Patients with active substance use disorder within the past 6 months (fulfilling the ICD 11 criteria for substance dependance) except caffeine and nicotine

2 Patients with any acute medical or surgical illness or receiving concomitant medications with contraindications to Fluoxetine

3 Pregnant and Lactating patients

4 Actively suicidal patients or those having high suicidal risk currently

5 Patients already on SSRI and/or Clomipramine for any other co morbid psychiatric condition

6 Patients receiving non pharmacological intervention like ERP or CBT
