Eligibility Criteria
Inclusion Criteria
- •(i) Patients admitted to an acute care hospital for heart failure
- •(ii) Patients whose symptoms have improved to the point where discharge is imminent
- •(iii) Patients with heart failure AHA staging Stage C or higher.
Exclusion Criteria
- •Patients with psychiatric disorders such as depression and schizophrenia, and patients diagnosed with delirium.
