- •Multimorbidity, i.e. the presence of at least two diseases requiring treatment,
- •Age = 70 years,
- •Stay in one of the participating clinics for in-patient treatment,
- •The patient's ability to consent,
- •written informed consent of the patient to participate in the study
排除标准
- •Age < 70 years,
- •There is no illness or only one illness requiring treatment,
- •no declaration of consent to participate in the study,
- •Presence of a language barrier,
- •Insufficient compliance (with uncontrolled alcoholism, drug addiction),
- •patient incapable of consent,
- •other reasons at the discretion of the investigator.
