To measure how cardiac Rehabilitation in heart failure is able to comply with the GBA demands for German DMP programs from August 2018:<br>Improvement in subcategories of Kansas City Cardiomyopathy Questionnaire (KCCQ) Score more than 20% during rehabilitation therapy (A/D) with stable results after 6m<br>Improvement of 2 points in Hospital Anxiety and Depression Scale (HADSD) Score A and HADSD Score D between A/D<br>Improvement of 20 Points or more in Barthel Score between A/D<br>Improvement of 40 Watt Performance in Ergometer Training between A/D<br>Increase in prognostic medication dose of 20% between A/D<br>Hospital Transfer /Admission lower than 5% between A/D
