Eligibility Criteria

Inclusion Criteria

  • •Consenting adult inpatients age > 18 years on treatment for at least 6 months period with 2 or more select chronic diseases such as
  • •a)Cardiovascular diseases (hypertension, coronary artery disease, peripheral artery disease, stroke, chronic kidney disease, heart failure)
  • •b)Musculoskeletal disorders (rheumatoid arthritis, osteoarthritis)
  • •c)Chronic respiratory disorders (COPD, asthma)
  • •d)Diabetes mellitus

Exclusion Criteria

  • •a)Patients who have psychiatric disorders, cognitive impairment or visual disturbance
  • •b)Patients who are unlikely to follow up at 6 months (Eg: living in another location)
  • •c)Critically ill patients
  • •d)Patients who have cancer, HIV or TB
  • •e)Patients who donâ??t know languages such as English, Kannada, Tamil, Telugu, Hindi and Marathi
  • •f)Patients or care givers without mobile phones or unable to read English messages
