Eligibility Criteria

Inclusion Criteria

  • •70 years or older,
  • •at least 5 drugs used continuously,
  • •treatment at Düsseldorf University Hospital,
  • •written consent of the patient

Exclusion Criteria

  • •legal care,
  • •inability to communicate due to language barriers or hearing loss,
  • •repeated inclusion due to recurrent treatments,
  • •visual impairment,
  • •cognitive impairment (minimum score of 4 on the Six Items Screening),
  • •inability to write,
  • •no written consent of the patient,
  • •instable clinical situation,
  • •patients with palliative care
