Inclusion Criteria
- •70 years or older,
- •at least 5 drugs used continuously,
- •treatment at Düsseldorf University Hospital,
- •written consent of the patient
Exclusion Criteria
- •legal care,
- •inability to communicate due to language barriers or hearing loss,
- •repeated inclusion due to recurrent treatments,
- •visual impairment,
- •cognitive impairment (minimum score of 4 on the Six Items Screening),
- •inability to write,
- •no written consent of the patient,
- •instable clinical situation,
- •patients with palliative care
