- •The study population consists of elderly persons (65 years and older) who are admitted to a hospital and \- during admission \- have a high risk of loss in function.
Exclusion Criteria
- •1\. Severe cognitive problems (MMSE \< 12\);
- •2\. Life expectancy shorter than 3 months;
