For medical record audit: Admitted to hospital with stroke
To be included in interventions and focus groups: health professional working on acute stroke unit
Exclusion Criteria
For medical record audit: No signs of stroke on admission to hospital, receiving palliative care, transferred to another hospital for acute management/investigation

Name Time Method Proportions of patients with stroke who received a documented assessment for rehabilitation. Data collected by assessor blinded to hospital allocation via medical record audit. Also collecting data regarding details of assessment (conducted by which health discipline, recommendation from assessment), [6 months post-commencement of intervention]

Name Time Method Proportions of patients who accessed stroke rehabilitation following discharge from acute hospital, collected via medical record audit by assessor blinded to hospital allocation. Details collected regarding rehabilitation setting (home, community, inpatient setting)[6 months post-commencement of intervention];Self-reported rehabilitation assessment and referral practices at each stroke unit. Factors identified by health professionals working on acute stroke units that influenced rehabilitation assessment and referral practices.
