Eligibility Criteria

Inclusion Criteria

  • •1\. Adult age more than 18 years olds and
  • •2\. capillary blood glucose in between 180 \- 400 mg/dl.

Exclusion Criteria

  • •1\) Patients need intravenous infusion insulin therapy
  • •2\)Continuous enteral feeding
  • •3\)Patients in a shock state with vasopressor and nothing per oral
  • •4\)Patient with Raynaud phenomenon, which interferes with capillary blood glucose measurement
  • •5\)Hypoglycemia prior admission
  • •6\)Chronic liver disease (child\-pugh score B and C)
  • •7\)History of insulin allergy or anaphylaxis
  • •8\)Surgical patients
  • •9\)Known pregnancy
  • •10\) Patient who cannot consent form or identify LAR within 48 hr
