1.Patients aged more 18 years with diagnosis of refractory septic shock
2.Within 24 hour of diagnosis of sepsis/septic shock; and
3.Who give consent for participation in the study, are eligible for inclusion in the study
1.Patients who are already receiving systemic corticosteroids at the time of screening.
2.Patients who require corticosteroids for longer duration such interstitial lung diseases, rheumatological diseases, exacerbation of asthma/COPD etc.
3.Immune compromised patients â?? cancers, HIV or prior history of corticosteroids use.
4.Patients who refuse to participate in the study.

Name Time Method Primary outcome: <br/ ><br>Overall reversal of the shock. <br/ ><br>Secondary outcomes: <br/ ><br>Reversal of shock on day-3 and day-7, in-hospital mortality, day-28 mortality, duration vasopressors requirement, and complication rates such as hyper-glycaemia, super infection and neuromuscular weakness. <br/ ><br>Timepoint: Day 3 <br/ ><br>Day 7 <br/ ><br>At the time of discharge
