1.Adults age 18 to 65 years with at least one elevated blood pressure measurement (as defined by study) identified by clinic-based screening,
2.Any clinic-based blood pressure measurement confirmed by another clinic-based, home, or ambulatory blood pressure measurement
3.First-degree relatives of hypertensive patients (father, mother, brothers or sisters, sons or daughters),
4.overweight (Body Mass Index of >= 25)
5.Dyslipidemia
1.Known secondary cause of hypertension that causes concern regarding safety of the protocol.
2.Arm circumference too large or small to allow accurate blood pressure measurement with available devices,
3.Cardiovascular event or procedure or hospitalization for unstable angina within last 3 months
4.A medical condition likely to limit survival to less than 3 years or a malignancy other than non-melanoma skin cancer within the last 2 years
5.Any factors judged by the clinic team to be likely to limit adherence to interventions.
6.Failure to obtain informed consent from participant
7.Any organ transplant
8.Pregnancy, currently trying to become pregnant, or of child-bearing potential and not using birth control
