Inclusion Criteria
Patients with chief complaint of dyspnea due to decompensated heart failure vascular type ( orthopnea, fine crackle on auscultation, raised JVP)
Exclusion Criteria
hypotension
recent myocardial infarction
history of allergy to TNG
patients who need intubation on admission
