Brief Summary: Karna naad which happens when Vata gets localized in the siras or channels which convey sounds. The sounds may vary and may be constant or intermittent. If left untreated, it may gradually give rise to hearing difficulties even for loud sounds and ultimately leads to deafness.

The prevalence of tinnitus in adults with hearing problems is very high (59 to 86%), and it is estimated that tinnitus is present in 50% of patients with sudden hearing loss, 70% with Presbycusis, and 50 to 90% with noise-induced hearing loss.The prevalence of tinnitus increases significantly with aging, but people of all ages experience tinnitus. Tinnitus is also experienced by those with normal hearing; 18% of tinnitus patients were reported to have normal hearing.

***Karnapoorana; -*** Karna being one of the Adhisthana of Vata- Dosha, Snehana becomes important to control the localized Increased Vata Dosha. Hence, Karnapoorana gains importance in the management of Karan naad Vata predominant diseases.*T*he ingratents of apamargkshar tail are murchit sesame oil, water , apamargkshar and apamarg. To preper properly medicatet oil murchit sessarm oil mejuring 1 letter , water treted with apamarkshar mejuring 4 latter and kalka of apamarg mejuring 250gm should be taken this. oil is used in karnanad (Noice inside the ear) and deffenss.

**Goghrita pana-** Goghrita having property â€œSamskarasya Anuvartanatvaâ€ When processed with Vatashamaka drugs and having Rasayana Guna becomes the best line of treatment for Karna naad. Hence Goghrita Paana was selected.

Karan naad is a Madhyama marga gata Vata Vyadhi in which Prakupita Vata dosha settles in Shabdavaha Shrotas as a result of Vimargagamana. Hence the drugs need to have Vatahara properties in order to normalize vitiated vata dosha. Snehana is described as an important procedure in treatment of vata dosha. Apamargkshar Taila is used for snehana in the form of karnapoorana. Apamargkshar Taila mainly contains ushna virya (65.78%) drugs and til taila is its main ingredient which is considered best for treatment of vata. Til Taila is an essential analgesic and useful for wound healing, skin, ear, dental and hair problems, useful in dryness by acting as Vatashamak. Local application of oil pack in ear reduces pain.

Inclusion Criteria

1.Patients presenting with symptoms of Karna naad irrespective of gender, caste and religion.
2.Patients in the age group of 25 to 75 years with intact Tympanic Membrane.

Exclusion Criteria

1.Congenital Deformity.
(Related to ear) 2.Perforation of Tympanic membrane.
3.Patient of Otosclerosis and fluid in the middle ear.
4.Blockage due to stenosis of external auditory canal/wax.
5.Pregnant, immune compromised patients.
6.Tinnitus due to non Otologic factors (Diabetes mellitus, Hypertension, Thyroid disease etc.) and glomus tumors, aneurysm of carotid artery, palatal myoclonus and tumors of auditory nerve.
8.Cases which required surgical intervention.
