Inclusion Criteria

Patients need long-term treatment with warfarin.
Patients who do not intend to have pre-planned surgery during the study.

Exclusion Criteria

Patients who have been treated with warfarin before hospitalization or before admission and whose INR values have been within the target range for the past three months
Patients suffering from chronic diseases in acute stages such as: cancer malignancies, liver cirrhosis, respiratory failure
Patients who are unable to follow the study
