Patients of either gender aged between 20-50 years, on invasive mechanical ventilation for >48 hrs. and put on spontaneous breathing trial (SBT) for the weaning process-
1. Fraction of inspired oxygen (FiO2 <0.5)
2. Positive end expiratory pressure (PEEP <5cm H2O)
3. Partial pressure of oxygen in arterial blood (PaO2)/FiO2 >200 mm Hg
4. Respiratory rate <35/minute
5. Tidal volume >5ml/kg
6. Minute ventilation < 10L/minute
7. No abnormality in electrolyte parameters
8. Absence of hemodynamic instability
9. Resolution of the initial cause of ventilator requirement
10. Patients awake with the ability to cough.
1. Refusal of consent
2. Patients with chest or abdominal trauma
3. Obese patients
4. Patients with localized fluid collections in the abdomen
5. Pregnant patients.
6. Known diaphragmatic paralysis or diaphragmatic surgery
