Inclusion Criteria
aged 18 or older with mechanical ventilation more than 48 hours, planed extubation and passed spontaneous breathing trial (SBT) and cough leak test, with ineffective cough measured by cough peak flow < 270 LPM.
Exclusion Criteria
pneumothorax, hemodynamic instability, hemoptysis, unfollow to command, gastrointestinal hemorrhage, bronchospasm, upper airway obstruction
