入选标准
- •1. Age > 18 years old and Follow up at Warfarin clinic of division of cardiothoracic surgery of Siriraj hospital.
- •2. Been using warfarin at least 3 months and INR stable around 2.00-3.00 at least 2 times.
- •3. Patient ≥ 60 years old with TMSE score > 23 or dementia with a care giver.
- •4. Patient or a care giver or relatives are able to use the application and having all-day internet
- •5. Patient or a care giver or relatives are able to understand and communicate with Thai language
- •6. Patient regularly follow up at warfarin clinic
排除标准
- •1. Unable to sign the informed consent legally
- •2. Been diagnosed of any serious diseases, such as coronary artery disease without treatment, stroke without treatment, cancer, severe liver or renal disease
