Inclusion Criteria

Patients treated by the PHV and its associated physicians, who perform a home dialysis procedure.

Exclusion Criteria

Existing health restrictions of the patient or the caregiver that make it difficult/prevent use of the app (e.g. cognitive restrictions, severe visual impairment, severe tremor, organizational problems, etc.) according to the assessment of the caregiver.

Observation period of the study max. 3 years / participant, baseline surveys at inclusion, 6 months, annually:<br>Mortality<br>Number of peritonitis / exit-site infections<br>(in particular also used antibiotics and pathogen), time until and between the occurrence of infection complications / hospitalization,<br>Number of physical doctor contacts<br>Number and type of digital consultations<br>Control of cardiovascular risk factors (blood pressure control (mean blood pressure), blood glucose control (HbA1c)),<br>dialysis and drug adherence<br>Quality of Life (Kidney Disease Quality of Life Short Form (KDQL-SF))<br><br>Target criteria of the substudy on usability, acceptance and patient participation with the app are measured by the endpoint of patient empowerment (SES6G (Self-Efficacy Scale/ Self-confidence in dealing with Chronic Disease)) and patient acceptance of the mobile application (qualitative evaluation of individual patient interviews as well as VAS scales).
