Inclusion Criteria

Thai patient is 30 to 70 years old
Previously diagnosed as Diabetes by a doctor and are currently taking diabetes medication
estimated glomerular filtration rate greater than or equal to 60 ml / min / 1.73 m2
must be treated prior at the Sub-district health office

Exclusion Criteria

Known case of Autosomal Dominant Polycystic Kidney Disease, lupus nephritis, obstructive uropathy, kidney stone, nephrotic syndrome or primary glomerular disease
Metastatic cancer (estimated life expectancy of less than 2 years)
AIDS
Pregnancy
Communication problems such as dementia, deafness
