Eligibility Criteria
Inclusion Criteria
- •Maori ethnicity (self – identified) AND
- •Community dwelling (not resident in rest home, private hospital or hospice) AND
- •55 years of age or older AND
- •Enrolled in a GP practice in Waitemata DHB AND
- •Taking four or more regular medicines for at least three months
Exclusion Criteria
- •Unable to give informed consent
