Maori ethnicity (self – identified) AND
Community dwelling (not resident in rest home, private hospital or hospice) AND
55 years of age or older AND
Enrolled in a GP practice in Waitemata DHB AND
Taking four or more regular medicines for at least three months

Exclusion Criteria

Unable to give informed consent
