SP1: a) Members of the DAK-G statutory health insurance, =60 years on December 31st, 2021, in need of care according to the German Social Code Book XI since December 31st, 2020, living in Hamburg and surrounding areas, cared for at home and able to consent.
b) Stakeholders from oral health care (family caregivers, GPs, dentists, nurses, and dental assistants).
SP2:
Study group: Data of insured persons of DAK-G health insurance company with incident ambulatory need of home care, =60 years and regular utilization of oral health care.
Control group: Data of insured persons of DAK-G health insurance company with no need of home care, =60 years and regular utilization of oral health care.
The data of the health-insured persons are obtained within the legal retention period (maximum six consecutive insured years).
SP3: Members of the DAK-G statutory health insurance, =60 years on December 31st, 2021, in need of care according to the German Social Code Book XI since December 31st, 2020, living in Hamburg and surrounding areas, cared for at home, able to consent and at least able to be transported in a seated position (necessary for the application of the orthopantomogram).
SP4: Experts from the associations (Bundesverband privater Anbieter sozialer Dienste, Landesverband Hamburg, Hamburgische Pflegegesellschaft, Hausärzteverband Hamburg, der Selbstverwaltung (Kassenzahnärztliche Vereinigung Hamburg, Kassenärztliche Vereinigung Hamburg), DAK-G und der Patient:innen-Organisationen (Patienteninitiative, Stiftung Patientenschutz) sowie der politisch-administrativ Verantwortlichen Ebene (Hamburger Behörde für Arbeit, Gesundheit, Soziales, Familie und Integration (Bereich: Gesundheit)).

Name Time Method SP1: <br>a) Oral health-related quality of life (OHIP-G14). <br>b) Not applicable. <br><br>SP2: Excess discontinuations of regular utilization of dental services, occurrence and/or severity of somatic diseases, health care costs. <br><br>SP3: Clinical oral health is assessed via the Oral Health Assessment Tool and ideally accompanied by a panoramic radiograph (objective oral health).<br><br>SP4: Implementation strategy (developed in the workshops), documented in a consortium working paper.
- Secondary Outcome Measures
Name Time Method SP1: a) and b): <br>generic health quality of life (EQ-5D), evaluation of barriers and facilitating factors in their perceived oral health, evaluation of the extent of cooperation between healthcare providers in different sectors (GP, D, nurses), reconstruction of the oral health care process in persons with a need for home care, description of the set of requirements for clinical pathways in oral health care as a root for SP4. <br><br>SP2: Other influencing factors such as person characteristics and regional differences will be examined.<br><br>SP3: Oral Health Impact Profile (OHIP-G14), questionnaire on the intensity of commitment to the GP (F-HaBi), clock drawing test and an Instrumental Activities of Daily Living scale (IADL). <br><br>SP4: Identifying indicators for intervention and outlining pathways for clinical care to improve oral health in persons in need of home care.
