Inclusion Criteria
1. The patients aged 60 years and older admitted at medical wards in Siriraj hospital.
2. Frail elderly patients according to the simple Frail Questionnaire Screening tool †Thai version
3. The patients who had a living area in Bangkoknoi or nearby this area.
Exclusion Criteria
1. The patients who lived in nursing homes.
2. Admitted patients at an intensive care unit
3. The patients were performed endotracheal intubation and using respirator more than 96 hours.
