Eligibility Criteria
Inclusion Criteria
- •1\. The patients aged 60 years and older admitted at medical wards in Siriraj hospital.
- •2\. Frail elderly patients according to the simple Frail Questionnaire Screening tool †Thai version
- •3\. The patients who had a living area in Bangkoknoi or nearby this area.
Exclusion Criteria
- •1\. The patients who lived in nursing homes.
- •2\. Admitted patients at an intensive care unit
- •3\. The patients were performed endotracheal intubation and using respirator more than 96 hours.
- •4\. End\-of\-life patients
