Inclusion Criteria
- •1. The patients aged 60 years and older admitted at medical wards in Siriraj hospital.
- •2. Frail elderly patients according to the simple Frail Questionnaire Screening tool †Thai version
- •3. The patients who had a living area in Bangkoknoi or nearby this area.
Exclusion Criteria
- •1. The patients who lived in nursing homes.
- •2. Admitted patients at an intensive care unit
- •3. The patients were performed endotracheal intubation and using respirator more than 96 hours.
- •4. End-of-life patients
