  • •Patients with at least 18 years of age, newly admitted in medical general ward in King Chulalongkorn Memorial Hospital

Exclusion Criteria

  • •1\. Newly admitted patients who refuse to join
  • •2\. Newly admitted patients who is previously admitted at other wards, including ICU
