- •a)Patients above the age of 60 years with at least one of the following Non-communicable diseases; diabetes, hypertensive, Coronary artery disease
- •b)Patients who give their consent
排除标准
- •a)Patients who are critically ill, not likely to survive through the study duration.
- •b)Patients who are living far away and are not likely to come for follow-up visits
- •c)Patients who are totally dependent on a care-giver. (e.g., bed-ridden patients, poor cognition)
- •d)Patients who do not have sufficient education to follow the study protocols
