Eligibility Criteria
Inclusion Criteria
- •a)Patients above the age of 60 years with at least one of the following Non\-communicable diseases; diabetes, hypertensive, Coronary artery disease
- •b)Patients who give their consent
Exclusion Criteria
- •a)Patients who are critically ill, not likely to survive through the study duration.
- •b)Patients who are living far away and are not likely to come for follow\-up visits
- •c)Patients who are totally dependent on a care\-giver. (e.g., bed\-ridden patients, poor cognition)
- •d)Patients who do not have sufficient education to follow the study protocols
