Inclusion Criteria
For the patients to be eligible to participate in this study, a subject must meet all the following criteria:
-Aged ≥18 years
Exclusion Criteria
Participants (patients) will be excluded during the study if he/she:
-Has severe cognitive impaired function. This will be evaluated by the professional that hands out the questionnaire. When the patient is not able to fill in the form, even with help from others, then the patient will be excluded

- Primary Outcome Measures
Name Time Method The main study parameter is the number (%) of patients screened, assessed, monitored, and treated according to the PG-SGA implementation plan. The number of referrals to several healthcare professionals is also a main study parameter. Additional main parameters are, the knowledge, attitude, acceptance, and perceived practice of the health professionals, and barriers and facilitators of the implementation.
