Any patient who has stayed in intensive care unit (ICU) and has been advised discharge from ICU by the physician will be included in the study
1.Patients who are being shifted out of ICU after Palliative Medicine consultation as part of Palliative care will be excluded.
2.Patients who are being shifted out of ICU due to discharge against medical advice (DAMA) due to any cause will be excluded from the study.
3.Any patient in whom the physician does not ask for the investigations like serum creatinine on the day of discharge from ICU will be excluded from the study.

- Primary Outcome Measures
Name Time Method Parameters which will be assessed as primary outcome measures are: Respiratory SpO2/FiO2 235, presence of scleral icterus, hypotension with mean arterial pressure 70mmHg, Glasgow Coma Scale score 9, urine output 200 mL in 24 hours or creatinine 5mg/dL. These parameters will be assessed each day after ICU discharge from day one to day three.Timepoint: From day one to day three of discharge from ICU to step down area - either ward or high dependency unit
