- •Patients aged 18\-75 years who are enlisted with three general practices
- •(1\) Polypharmacy patients using five or more different types of medication ,
Exclusion Criteria
- •Severe psychiatric and cognitive problems, language problems.
- •By GP expected harm for patient or others from having access to records in general practice.
