1.Patients age from 30yrs to 75yrs
2.Male or female with a diagnosis of type 2 diabetes mellitus with or without co morbitites
3. Patients who are giving consent to participate
1.Patients have major complications which would interfere with self-care management behaviors (e.g. legally blind, severe stroke, or undertaking kidney dialysis etc.) patients’ health records will be checked for the presence of any major DM complications.
2.Patients with cognitive impairment
3.Patients who are not able to communicate,
4.Patient with any mental or psychiatric illness, patients’ health records will be checked.

Name Time Method 1.Proportion changes in self-care practices among people living with diabetes. <br/ ><br> <br/ ><br>2.Mean Changes in FBG (Fasting Blood Glucose)/RBS (Random blood sugar). <br/ ><br> <br/ ><br>3.Mean changes in HbA1c and Lipid Profile. <br/ ><br> <br/ ><br>4.Means intake of Fat, Sugar, Salt, Vegetables and Fruit in grams per day. <br/ ><br>Timepoint: Time point: Baseline,3 months, 6 months and 1 year
