- Primary Outcome Measures
Name Time Method There will be improvement in the quality of transfer process from critical care post implementation of the transfer model. There will be a difference between the pre & post implementation score. Caregivers will be learning more details abouts the patient current condition and the better way to take a good care of them. Family satisfaction rate will be more about the transfer process. Overall a better transfer care will be provided to all the transfer patients from ICU to ward. <br/ ><br> <br/ ><br>Timepoint: at interval of 15 days post discharge
