Inclusion Criteria

Eligible individuals were those who met the following criteria: patients diagnosed with Heart Failure, aged 18 years or older; in follow-up, with a smartphone with Android operating system; taking medication; who had adequate health literacy, according to the Eighth-item Health Literacy Scale (19 points); and oriented in time and space. In order to evaluate the orientation of individuals, the following questions were asked: What is your name? How old are you? What day, month, year and location are we in? What city do you live in?

Exclusion Criteria

individuals who, due to clinical decompensation, resulted in previous hospitalization in the last 30 days; who were listed on the heart transplant queue and individuals planning myocardial revascularization or interventional procedure such as Transcatheter aortic valve implantation
