- •Aged 60 years or older.
- •Diagnosed with first-ever ischaemic stroke confirmed by a physician.
- •Medically stable and preparing for hospital discharge.
- •Able to communicate in Thai.
- •Six Item Cognitive Impairment Test (6CIT) score under
- •National Institutes of Health Stroke Scale (NIHSS) score 0 to
- •Able to use the LINE application independently or with assistance from a caregiver.
- •Willing to provide written informed consent.
排除标准
- •History of recurrent stroke.
- •Severe cognitive impairment or inability to provide informed consent.
- •Severe aphasia or communication problems preventing participation.
- •Severe psychiatric illness.
- •Unstable medical condition requiring intensive treatment.
- •Participation in another intervention study that may influence the study outcomes.
