1) Diagnosed hypertension disease by physician within 5 years,
2) Aged between 18-59 years old
3) Who has poorly controlled blood pressure level of systolic and diastolic blood pressure between 140/90 -180/100 mmHg in the last four weeks
4) Participants are able to communicate, read and write in Thai language.
5) Who has no serious illness or complications by hypertension such as stroke, or alteration of consciousness.
1) co-morbidities not related to vision and hearing loss, musculoskeletal problem, renal failure, heat failure 2) health status deteriorates by condition such as stroke, or alteration of consciousness. Moreover, the participants who are incapable of completing the three phases of the study were excluded.
