1.Patients who have used health services and been recorded in health records in the studied sub-district health centres, both intervention and control sub-districts.
2.Patients who have at least one of the following chronic mental illnesses: schizophrenia, bipolar disorder, depressive disorder, and alcohol and substance use disorders
3.Patients who have at least one of the following chronic non-communicable diseases: cancers, cardiovascular, diabetes, hypertension and chronic respiratory diseases
4.Patients who have experienced an acute life crisis
1.Patients who move out of the studied areas during the study period
2.Patients who have too many symptoms to cooperate in data collection
3.Patients who cannot communicate using the Thai language
