- •1.Patients who have used health services and been recorded in health records in the studied sub-district health centres, both intervention and control sub-districts.
- •2.Patients who have at least one of the following chronic mental illnesses: schizophrenia, bipolar disorder, depressive disorder, and alcohol and substance use disorders
- •3.Patients who have at least one of the following chronic non-communicable diseases: cancers, cardiovascular, diabetes, hypertension and chronic respiratory diseases
- •4.Patients who have experienced an acute life crisis

- •1.Patients who move out of the studied areas during the study period
- •2.Patients who have too many symptoms to cooperate in data collection
- •3.Patients who cannot communicate using the Thai language
