Investigators
Eligibility Criteria
Inclusion Criteria
- •1\.Patients who have used health services and been recorded in health records in the studied sub\-district health centres, both intervention and control sub\-districts.
- •2\.Patients who have at least one of the following chronic mental illnesses: schizophrenia, bipolar disorder, depressive disorder, and alcohol and substance use disorders
- •3\.Patients who have at least one of the following chronic non\-communicable diseases: cancers, cardiovascular, diabetes, hypertension and chronic respiratory diseases
- •4\.Patients who have experienced an acute life crisis
Exclusion Criteria
- •1\.Patients who move out of the studied areas during the study period
- •2\.Patients who have too many symptoms to cooperate in data collection
- •3\.Patients who cannot communicate using the Thai language
