Inclusion Criteria
- •Adult patients aged 18 to 60 years and older patients diagnosed with NSTE ACS with a GRACE score less than 130 based on age and stable clinical symptoms, and scheduled for elective PCI post-discharge. Secondly, no cognitive impairment indicated by a score over 23 on the Thai Mini-Mental State Examination TMSE. Thirdly, no or mild anxiety and depression indicated by a score less than 11 on each subscale of the Thai version of the Hospital Anxiety and Depression Scale Thai HADS. Lastly, capable of seeing, hearing, communicating in Thai, standing for weight measurement, and transmitting data via a mobile application independently or with caregiver assistance.
Exclusion Criteria
- •Undergoing emergency surgery for any reason, or experiencing severe complications requiring hospitalization that preclude participation in program activities.
- •Having life-threatening conditions (e.g., acute pulmonary edema, recurrent ACS, stroke, advanced-stage cancer) or planned surgery within 2 months.
- •Requiring emergency PCI outside of the original treatment plan.
- •Having more than one primary caregiver involved in assisting with the study activities for a single patient.
