Eligibility Criteria

Inclusion Criteria

  • •1\. New York Heart Association(NYHA) classification â?? II to IV
  • •2\. Diagnosed with Heart Failure with reduced ejection fraction.
  • •3\. Left Ventricular ejection fraction of \< 45%
  • •4\. Patients who are willing to consent for the study and able to read/understand the Kannada language.
  • •5\. Ability and willingness to operate the mobile phone for text message and respond for telephonic calling

Exclusion Criteria

  • •1\. Patients with cognitive impairment
  • •2\. Patients who are critically ill unable to perform self\-care activities.
  • •3\. Patients/ caregivers who do not use a mobile phone
  • •4\. Patients with other comorbidities having limited life expectancy such as chronic obstructive pulmonary, asthma, cancer, chronic kidney disease
