Eligibility Criteria
Inclusion Criteria
- •1\) Older adults aged 65 years and above residing in the local community.
- •2\) Individuals who have received long\-term care assessment and have been classified into grades 1 to 5, with a long\-term care certification score of 45 or above.
- •3\) Individuals who have experienced proxy prescription or unmet medical needs due to reasons such as mobility impairment within the past 3 months.
- •4\) Individuals with decreased ability to perform activities of daily living (Clinical Frailty Scale grade 4 or above) who have a need for home care services.
- •5\) Individuals requiring ongoing healthcare management and medical intervention from physicians due to multiple chronic conditions or other reasons.
Exclusion Criteria
- •1\) Individuals registered in hospice care due to terminal illness or those predicted to have a survival period of less than one month.
- •2\) Individuals who prefer facility\-based long\-term care benefits over home\-based care benefits.
- •3\) Individuals who, despite receiving sufficient explanation regarding research participation, do not provide written consent.
