•Patients with age 18 years or older; diagnosis of Hypertension according to the criteria of the Brazilian Guideline of arterial hypertension; use at least one medication for Hypertension for at least six months; purchase at least one medication for Hypertension at the community pharmacy in the municipality of Alegre/ES in the last six months.

•patients with plans to move from the municipality during the study period; people who do not have autonomy over their health and use of their own medicines; pregnant women, women with the intention of becoming pregnant during the execution of the study and/or lactating women; patients with terminal disease (eg terminal cancer; chronic kidney disease on dialysis); individuals who do not attend three consecutive pharmaceutical consultations; patients treated by other clinical pharmacy services; patients with a history of myocardial infarction or stroke in the last six months; individuals who are participating in other studies that assess impacts of health interventions on cardiovascular parameters and glycemic control.
