Inclusion Criteria
- •1. Outpatients with ischemic stroke treated at the Stroke Clinic and the Neurology Clinic of Chulalongkorn Hospital within 1 year after discharge from ward of Chulalongkorn Hospital
- •2. Aged 18 years and above.
- •3. Patients can understand and communicate in Thai language
- •4. Willing to participate in the research.
Exclusion Criteria
- •1. Patients who have not received continuous treatment at Chulalongkorn Hospital.
- •2. Patients with cognitive impairment due to ischemic stroke.
