Inclusion Criteria

First and second mandibular molars; diagnosis of necrosis with periapical lesion; completely closed apices; in need of endodontic treatment; patient between 18 and 80 years old; patients with painful symptoms.

Exclusion Criteria

Patients who used antibiotics, immunosuppressive and anti-inflammatory drugs in the last three months; teeth with root fractures, previous endodontic treatment, periodontal pocket, anatomical complexities, coronary fracture or very extensive cavities; pregnant and lactating patients; patients with systemic and oral cavity diseases that could influence the natural course or treatment of the disease; patients with systemic conditions that need prophylactic antibiotic coverage; patients who did not agree to participate or sign the IC.

The clinical and radiographic evolution evaluation, verified by the periapical index method (scoring system to assess periapical bone repair), high rates of periapical healing (97%) were observed in both therapeutic groups in the 12-month follow-up.;In the evaluation of clinical and radiographic evolution, the comparison between the control group and the group using photodynamic therapy (TF group), verified by the periapical index method (scoring system to assess periapical bone repair), no additional benefit was observed over conventional therapy alone in promoting periapical healing. At 12 months, the frequency of the lowest scores (1 and 2) in the control group was 93.3% and in the TF group, 100%.
