Inclusion Criteria
The patients are:
- 18 years or older.
- competent to make decisions concerning their health
- Have =2 out of the 3 following problems: psychiatric, chronic disease or social problem.
Have more than average acute medical care usage ( =2 times per year).
Exclusion Criteria
The exclusion criteria done by the general practitioner:
• The patient is terminal.
• The patient has dementia and is unable to communicate effectively.
• the patient is lacking decision making capacity (concerning their health).
• The patient doesn’t give permission to participate in the study.
