1. Persons diagnosed with HF for at least 6 months by the cardiologist.
2. NYHA functional class II to III at the time of recruitment, which the cardiologist confirmed based on NYHA functional class criteria.
3. Aged 40 years or older because of the risk of developing HF increases in this age (Benjamin et al., 2018; Virani et al., 2020).
4. Stable condition of comorbidities in terms of able to control blood pressure and blood sugar, no chronic kidney disease stage IV and V.
5. No impaired vision or hearing.
6. No cognitive impairment by screening with the Thai Mental State Examination (TMSE), the TMSE should be greater than 23 points.
7. Has a mobile phone and can be contacted by LINE application.
8. Able to read, write, and communicate in Thai language.
1. The participants cannot continue in all sessions of intervention.
2. The participants have severe symptoms or complications from heart or comorbidity during the period of the program.
3. The participants have worsening conditions or transfer to the intensive care unit or death before the end of the program.
