Inclusion Criteria

Patients who have been on warfarin for at least 3 months.
The patient's age is 18 to 60 years.
No hearing, visual or tactile disorders.
Ability to speak Persian.
Have at least literacy.
Have an Android smartphone.
Willing to participate in the study.

Exclusion Criteria

Have Simple mobile phone.
Be illiterate.
Not willing to participate in the study
Participants must be under 18 or over 60 years of age.

Primary Outcome Measures

Medication adherence was used in this study to measure medication adherence using the Moriskey adherence Scale. Timepoint: Medication adherence was measured before, 30, 60, and 90 days after the intervention. Method of measurement: Medication adherence was used in this study to measure medication adherence using the Moriskey adherence Scale. The overall scores range from 0 to 8 with a score of 8, high medication adherence, a score of 6 and 7, a moderate adherence to a score. Less than 6 are considered low.;The rate of readmission is due to Warfarin side effects using a checklist. Timepoint: Readmision rate was measured before, 30, 60, and 90 days after the intervention. Method of measurement: In this study, patients' records and self-reports were used to measure readmission rates for warfarin complications, then reviewed by a researcher-made literature review checklist and then the number of readmissions recorded, And it will be calculated.
