Diagnosis of heart failure by physician
No known mental symptoms
consent to attending the plan
having a telephone line
having a companion
patient at the 2nd and 3rd care level. .
At least six months of diagnosis
Exclusion Criteria
patient at 4rd care level.
The patient should not be satisfied with the plan
