- •Individuals/Persons aged >=18 years, diagnosed for non-communicable diseases for at-least one year such as diabetes, hypertension and provided consent for the study will be included
排除标准
- •Participants who cannot able to read Hindi/English.
- •Patients with end-organ complication related to NCD
- •Participants with intellectual disability
- •Participants who didnâ??t give consent to the study
