Inclusion Criteria
- •Individuals/Persons aged \>\=18 years, diagnosed for non\-communicable diseases for at\-least one year such as diabetes, hypertension and provided consent for the study will be included
Exclusion Criteria
- •Participants who cannot able to read Hindi/English.
- •Patients with end\-organ complication related to NCD
- •Participants with intellectual disability
- •Participants who didnâ??t give consent to the study
