Inclusion Criteria

  • Adults who are referred by primary care to a combined lifestyle intervention or physical activity in which the Care Sports Connector is involved.

Exclusion Criteria

  • Insufficient understanding of the Dutch language.
  • A negative advice from the Par\-Q
